Ocasional moderation voting glitch?

I've experienced something very strange, it's a glitch with voting. Sometimes after I vote on a bunch of posts, there will be one at the very bottom of the page I'm unable to accept or reject vote. It's happening with the vote in the image below. I would send a screen recording but I'm unable to unfortunately. Has anybody else experienced this?

Also, sorry for the lackluster description, it's the best I could do!

wait, there's a vote limit? why would that be beneficiary?

There's a limit? If that's what it is, then it should give you a dialog saying you're out of votes instead of flat out not working.
