Repair Checklist for Customers

I'm starting a small repair shop and I'm just wondering if anyone has any info on what a checklist and customer agreement would look like so I can properly cover myself and my business. I want to know what kind of issues I should address to avoid any possible conflicts. I will be seeking legal aid but I would like a template to start out with. Any help on this is highly appreciated.

First of all, congrats on starting a repair shop! I wish you the best of luck! To get ya started, have some form of documented functionality test before and after the repairs have been completed, and perform them in front of your customers and be sure to make them aware of any underlying issues like a malfunctioning camera and such.
